Undergraduate Forms
Most of the forms below are available in either microsoft word or adobe pdf versions. The first version has the advantage that it can be filled in electronically though most forms will have to be printed since they require a signature.

- Special Circumstances Form - word or pdf
- Any student who experiences special circumstances which might adversely affect their academic performance should complete this form. The deadlines for submitting information about mitigating circumstances to the Examiners' Boards are:
- First Semester Examinations: FRIDAY, 3 FEBRUARY 2012,
- Second Semester Examinations: FRIDAY, 8 JUNE 2012.
The form should be submitted to the Teaching and Learning Office (Room G.204, Alan Turing Building). The student will be given an acknowledgement of receipt. - See also the University guidance to students for further details.

- Interruption and/or Repeat of Studies - word or pdf
- Any student thinking of interrupting or suspending their studies, must discuss their plans with their Academic Advisor as soon as possible. They must then see the Senior Tutor, Dr. R. M. Thomas, (Room 1.108, Alan Turing Building) for approval. They will then complete this form and hand it in to the Senior Tutor.

- Change of options form - word or pdf
- Any Student who wish to change their options after the end of Week 2 of a Semester must complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).
- Outside Course Unit form - pdf
- Students who wish to take an outside course unit need written permission from the Senior Tutor (unless the outside course unit you want to take is one of those listed as an approved outside course in the School of Mathematics Undergraduate Student Handbook). You must complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).
- Project Registration Form - word or pdf
- When a student has chosen a Third or Fourth year project topic and found a project supervisor they must use this form to register for the project.
- Project Completion form - word or pdf
- When a student has finished their Third or Fourth year project they must use this form when submitting the project. They will receive a photocopy of the form as a receipt. The deadline is the first day of the appropriate examination period.
- Student Withdrawal Form -

- Any student who wishes formally to withdraw from the University should fill in this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).

- Transfer of Degree Programme Form - word or pdf
- Any student who wishes to transfer to a different degree programme, both within the School of Mathematics and also to one in a different School, should complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).
- Examinations Entry Correction Form - word or pdf
- Any student for whom the exam timetable is wrong, should fill out this form.
The reason the timetable is wrong may be that the student is not entered for an exam for which they should be or they have been entered for an exam for a course unit they have dropped.
The form should be returned to the Teaching and Learning Office (Room G.204, Alan Turing Building).

- Transcript Request Form - word or pdf
- Any student who wishes to obtain an unofficial transcript (not stamped) should use this form. You may also obtain a transcript from the University, but this will take 6-8 weeks.
- Academic Appeals
- Any Student who wishes to appeal against academic decisions of the School should make the appeal in writing using the Academic Appeal Form. They should look at the relevant information in Regulation XIX Academic Appeals.
The reason for appealing may be against the result of an exam, the dealing with disciplinary matters or the outcomes of requests to interrupt and/or repeat a year. - See the StudentNet for more details.

- Letter request Form - word or pdf
- Any student requiring a letter proving that they are a student in the School of Mathematics or at the University should fill in this form.
The letter may be required, for instance, to open a bank account,
