Undergraduate Forms

Most of the forms below are available in either Microsoft word or Adobe PDF versions. The first version has the advantage that it can be filled in electronically though most forms will have to be printed since they require a signature.

  • Interruption and/or Repeat of Studies - Undergraduate Interruption Form [Word]   or Undergraduate Interruption Form [PDF] 
    • Any student thinking of interrupting or suspending their studies, must discuss their plans with their Academic Advisor as soon as possible. They must then see the Senior Tutor, Dr. R. M. Thomas, (Room 1.108, Alan Turing Building) for approval. They will then complete this form and hand it in to the Senior Tutor.

  • Change of options form - Change of Options [Word] or Change of Options [PDF] 
    • Any Student who wish to change their options after the end of Week 2 of a Semester must complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).

  • Outside Course Unit form - Outside Course Unit [PDF] 
    • Students who wish to take an outside course unit need written permission from the Senior Tutor (unless the outside course unit you want to take is one of those listed as an approved outside course in the School of Mathematics Undergraduate Student Handbook). You must complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).

  • Project Registration Form - Project Registration Form [Word]  or Project Registration Form [PDF] 
    • When a student has chosen a third or fourth year project topic and found a project supervisor they must use this form to register for the project.

  • Project Completion Form - Project Completion Form [Word]   or Project Completion Form [PDF]
    • When a student has finished their third or fourth year project they must use this form when submitting the project. They will receive a photocopy of the form as a receipt. The deadline is the first day of the appropriate examination period.

  • Student Withdrawal Form - Student Withdrawal Form [PDF] 
    • Any student who wishes formally to withdraw from the University should fill in this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).

  • Transfer of Degree Programme Form - Transfer of Degree Programme Form [Word]  or Transfer of Degree Programme Form [PDF] 
    • Any student who wishes to transfer to a different degree programme, both within the School of Mathematics and also to one in a different School, should complete this form and then submit it (in person) to the Senior Tutor (Dr. R. M. Thomas, Room 1.108, Alan Turing Building).

  • Examinations Entry Correction Form - Examinations Correction Form [Word]  or Examinations Correction Form [PDF] 
    • Any student for whom the exam timetable is wrong, should fill out this form. 
      The reason the timetable is wrong may be that the student is not entered for an exam for which they should be or they have been entered for an exam for a course unit they have dropped.
      The form should be returned to the Teaching and Learning Office (Room G.204, Alan Turing Building).

  • Transcript Request Form - Transcript Request Form [Word]  or Transcript Request Form [PDF] 
    • Any student who wishes to obtain an unofficial transcript (not stamped) should use this form. You may also obtain an official transcript from the University, but this will take 6-8 weeks.

  • Academic Appeals
    • Any Student who wishes to appeal against academic decisions of the School should make the appeal in writing using the Academic Appeal Form. They should look at the relevant information in Regulation XIX Academic Appeals.
      The reason for appealing may be against the result of an exam, the dealing with disciplinary matters or the outcomes of requests to interrupt and/or repeat a year.

  • Letter Request Form - Letter Request Form [Word]  or Letter Request Form [PDF]  R
    • Any student requiring a letter proving that they are a student in the School of Mathematics or at the University should fill in this form. 
      The letter may be required, for instance, to open a bank account.
      The form should be returned to the Teaching and Learning Office (Room G.204, Alan Turing Building).
