The forms below are available either electronically on-line, Microsoft word or Adobe PDF versions. The online electronic version has the advantage that it can be filled in electronically with an automatic email receipt, though the other forms will have to be printed and handed into Reception, Alan Turing Building, or emailed to the Teaching and Learning Office email: mathematics@manchester.ac.uk.

Outside course unit approval form

Before completing this form you must read the information under your programme specification to see if you are eligable to take outside course units.

Further details and a list of approved outside course units is available under the section on Outside Course Units, in the Schools Undergraduate Student Handbook. In addition, outside course units recommended as part of your programme of study are listed under the programme specifications. You can enrol onto a recommended outside course unit through the course unit selection process.

Students interested in taking a LEAP course unit offered by the University Language Centre will need to contact the Administrator(s) in the Language Centre to be assessed before completing the section on LEAP of this form before approval from the School.

Students who wish to take an outside course unit need written permission from the lecturer of the new course (unless the outside course unit you want to take is one of those listed as recommended outside course units in the School of Mathematics Undergraduate Student Handbook.

Students who complete the Outside Course Unit Approval online form will be contacted by email if the request is approved. 

Application form for year 3 mathematics students wishing to enrol onto a level 2 mathematics course unit

This application form is to be completed by year 3 mathematics students who wish to enrol onto a level two mathematics course unit as part of their programme of study.

Subject to approval, year 3 students may include a maximum of 20 credits of level 2 mathematics options depending upon their degree programme. If you wish to enrol onto a level 2 mathematics option, having checked your programme requirements under the programme specifications and the timetable of the course units, please complete this application form for approval.  

However, please note the decision to enrol onto a level 2 course unit will not be decided until the majority of year 2 students have completed their unit selections.  Therefore we recommend that you enrol onto a level 3/4 course unit option which can be substituted for the level 2 course unit.

Approval form for year 2 mathematics students wishing to defer MATH20201 or MATH20701

This approval form is to be completed by year 2 mathematics students wishing to defer MATH20201 or MATH20701 until year 3.

Subject to approval, year 2 students may defer a maximum of 10 credits of level 2 mathematics options depending upon their degree programme. Before completing the form please check the criteria for your programme of study.

Approval form for an uneven semester split

This application form is to be completed by mathematics students wishing to take an uneven split in semesters 1 and 2.

Subject to approval students can request permission to take an uneven split of credits in semester 1 and 2 totalling 120 credits, for example 70:50 split.  Before completing the form please check the criteria for your programme of study.

Mitigating circumstances application form

Mitigating Circumstances - The Mitigating Circumstances Form should be completed by any student who experiences unpreventable or unforeseeable circumstances that could have a significant adverse effect on their academic performance. Please referr to the mitigating circumstances page for full details about this procedure and applications.

Academic appeals form

Any student who wishes to appeal against academic decisions of the School should make the appeal in writing using the Academic Appeal Form. They should look at the relevant information in Regulation XIX Academic Appeals.

The reason for appealing may be against the result of an exam, the dealing with disciplinary matters or the outcomes of requests to interrupt and/or repeat a year  See the StudentNet for more details. 

Application form to withdrawal from your studies

This application form should be used when a student wishes formally to withdraw from the University. All information on this form is treated as STRICTLY CONFIDENTIAL and will not be divulged in personalised form outside the University.

Before withdrawing, it is important that you consider the implications. Help and advice can be obtained from your academic advisor/year tutor or programme director, other academic staff in the School and the Students Union. 

Once the form has been completed, you will be informed by the School on completion of your withdrawal. Please note that students may be liable for fees, refund of grants, loans or other awards and it may delay transfer to other programmes of study at other universities or colleges.

Other Forms

Interruption and/or Repeat of Studies Form

Any student thinking of interrupting or suspending their studies, must discuss their plans with either their Academic Advisor, Year Tutor or Programme Director, as soon as possible. You will then need to complete this form and hand it in to the Teaching and Learning Office. See also Student Guidance and also the StudentNet.

Change of Options Form 

Any Student who wish to change their options after the end of Week 2 of a Semester must complete this form and then submit it to the Teaching and Learning Office.

Transfer of Degree Programme Form 

Any student who wishes to transfer to a different degree programme, both within the School of Mathematics and also to one in a different School, should complete this form and then submit it to the Teaching and Learning Office.

Examinations Entry Correction Form 

Any student for whom the exam timetable is wrong, should fill out this form. 
The reason the timetable is wrong may be that the student is not entered for an exam for which they should be or they have been entered for an exam for a course unit they have dropped.
The form should be returned to the Teaching and Learning Office.

Transcript Request Form 

Any student who wishes to obtain an unofficial transcript (not stamped) should use this form. You may also obtain an official transcript from the University.

Letter Request Form 

Any student requiring a letter proving that they are a student in the School of Mathematics or at the University should fill in this form. The letter may be required, for instance, to open a bank account.
