The forms below are available either electronically on-line, Microsoft word or Adobe PDF versions. The online electronic version has the advantage that it can be filled in electronically with an automatic email receipt, though the other forms will have to be printed and handed into Reception, Alan Turing Building, or emailed to the Teaching and Learning Office email: firstname.lastname@example.org.

This form should be used if you require permission to:
- Defer MATH20201 and/or MATH20701 in year 2
- Take a course unit that has a timetable clash
- Take a course unit without the required pre-requisite
- To be added to a waiting list of a course unit that is full
- To study an uneven semester split
Before completing the form you must check that it is permitted as part of your programme requirement. The form will open in line with the course unit selection process timeline.
Before completing this form you must read the information under your programme specification to see if you are eligable to take outside course units.

Students who complete the Outside Course Unit Approval online form will be contacted by email if the request is approved.
This application form is to be completed by year 3 mathematics students who wish to enrol onto a level two mathematics course unit as part of their programme of study. The form is available from July until September to enrol on both semester 1 and semester 2 course units. Note some level 2 course units may not be available, further details are available from the course unit selection website.
Subject to approval, year 3 students may include a maximum of 20 credits of level 2 mathematics options depending upon their degree programme. If you wish to enrol onto a level 2 mathematics option, having checked your programme requirements under the programme specifications and the timetable of the course units, please complete this application form for approval.
However, please note the decision to enrol onto a level 2 course unit will not be decided until the majority of year 2 students have completed their unit selections. Therefore we recommend that you enrol onto a level 3/4 course unit option which can be substituted for the level 2 course unit.
The MATH30002 Mathematics Education course unit module is 100% assessment which means there is a limit on the number of places available. Students interested in enrolling onto this course unit will need to complete an on-line registration form whereby students will need to write no more than 100 words as to why they wish to take this course unit.
It is recommended that you enrol in the first instance on an alternative course unit which you can drop when a place has been confirmed on the MATH30002 course unit.
The final deadline to register is Friday 1st February 2019 in semester 2. However we suggest you complete the form from the 13th July to make sure you have a secured place come semester 2.
Mitigating Circumstances - The Mitigating Circumstances Form should be completed by any student who experiences unpreventable or unforeseeable circumstances that could have a significant adverse effect on their academic performance. Please referr to the mitigating circumstances page for full details about this procedure and applications.
Any student who wishes to appeal against academic decisions of the School should make the appeal in writing using the Academic Appeals Form (word version) They should look at the relevant information in Regulation XIX Academic Appeals.
The reason for appealing may be against the result of an exam, the dealing with disciplinary matters or the outcomes of requests to interrupt and/or repeat a year See the StudentNet for more details.
This application form should be used when a student wishes formally to withdraw from the University. All information on this form is treated as STRICTLY CONFIDENTIAL and will not be divulged in personalised form outside the University.
Before withdrawing, it is important that you consider the implications. Help and advice can be obtained from your academic advisor/year tutor or programme director, other academic staff in the School and the Students Union.
Once the form has been completed, you will be informed by the School on completion of your withdrawal. Please note that students may be liable for fees, refund of grants, loans or other awards and it may delay transfer to other programmes of study at other universities or colleges.
Interruption and/or Repeat of Studies Form
Any student thinking of interrupting or suspending their studies, must discuss their plans with either their Academic Advisor, Year Tutor or Programme Director, as soon as possible. Change of Options Form - after week 2 of a semester
Any Student who wish to change their options after the end of Week 2 of a Semester deadline must complete this form and then submit it to the Teaching and Learning Office for approval.
Transfer of Degree Programme Form
Any student who wishes to transfer to a different degree programme, both within the School of Mathematics and also to one in a different School, should complete this form and then submit it to the Teaching and Learning Office.
Transcript Request Form
Any student who wishes to obtain an unofficial transcript (not stamped) should use this form. You may also obtain an official transcript from the University.
Letter Request Form
Any student requiring a letter proving that they are a student in the School of Mathematics or at the University should fill in this form. The letter may be required, for instance, to open a bank account.
